Integrating Care for Co-occurring Mental Illness and Chronic Disease to Improve Client Outcomes
Our thanks to Tracy Tinker, RN, MSN, who presented a training program on the Intersection of Chronic Disease and Behavioral Health recently attended by 40 CLM staff. A Nurse Case Manager at the Healthcare for the Homeless program of Catholic Medical Center, Tracy is a passionate advocate for client-centered goal setting for clients with diabetes/pre-diabetes helping them to successfully achieve self-management goals. Topics covered included an overview of chronic disease management related to diabetes, hypertension, and dyslipidemia (high cholesterol).
Our goal in the coming months and years is to move forward with our agency’s healthcare integration planning. The focus of that work will be to better identify and integrate treatment planning for clients with co-occurring mental illness and chronic disease conditions. Through these efforts, we anticipate better outcomes for these higher risk individuals.
