Center for Life Management (CLM) provides the area’s most comprehensive array of outpatient mental health services for Southern New Hampshire. Our Integrated Care Program is a fast growing highly innovative approach to whole healthcare that encompasses the integration of treatment modalities for mental and physical health needs. The Integrated Care team provides a full wrap-around care model that is necessary to support individuals with addressing both their significant mental and chronic physical health conditions. The Integrated Care Whole Health Clinician provides initial and ongoing diagnostic assessment, treatment planning, therapy, and crisis management to help client’s improve medical and mental health symptoms and behaviors that otherwise interfere with daily functioning and personal wellness goals. This is a full time, 35 or 40 hour per week position.
Major Functional Areas and Description of Duties of the Integrated Care Behavioral and Physical Clinician.
- Provide initial and ongoing individual diagnostic assessment that incorporates physical health diagnosis and symptoms to determine client’s strengths, needs, resources, and objectives;
- Formulate treatment plans that are collaborative with primary and specialty care specifying presenting problems, objectives, interventions, and desired outcomes;
- Complete assessments for eligibility determination for State funded services;
- Provide individual and group therapy in the office, community and/or remote via telehealth;
- Provide counseling support as deemed appropriate for the objectives, interventions and desired outcomes identified in the client’s treatment plan to develop, apply, and reinforce coping strategies to reduce and/or ameliorate symptoms and behaviors that interfere with functioning and goal achievement;
- Collaborates with families and supports by providing family training and counseling services as needed to educate natural supports and promote how to best encourage the client’s resiliency and recovery toward treatment goals;
- Provide psycho-education using the evidence-based practice of Integrated- Illness Management and Recovery (I-IMR) as needed to support goal and coping skill development;
- Provide crisis management and intervention when clients experience acute exacerbation of symptoms to ensure their safety within the home and/or community;
- Adhere to and uphold the practices specific to the Integrated Care Model specific to support the authenticity of program implementation;
- Work collaboratively with other treatment team members, natural supports, and external providers to ensure a wrap-around approach is provided for continuity of client care;
- Develop training opportunities in an area of expertise to educate internal and external colleagues;
- Provide set direct client care hours each week (eligible for incentive pay upon exceeding client care expectation);
- Engage in appropriate consultation meetings and committees, both internally and externally, as determined by client needs and/or supervisor;
- Function professionally and in a manner that protects the integrity, confidentiality and rights of all clients;
- Execute all required services and timely documentation in accordance with agency policies and federal/state standards;
- Maintain professional memberships, licenses, accreditation, certifications, etc.;
- Represent CLM in a professional and appropriate manner in all settings;
- Perform all other duties as assigned by supervisor.
Qualifications and Skills
- Master’s degree required in mental health field (social work, clinical mental health counseling, psychology etc.)
- Licensed Clinical Mental Health Counselor (LCMHC), Licensed Independent Clinical Social Worker (LICSW) or licensed eligible master’s level clinician preferred;
- Knowledge of chronic co-morbid physical health conditions and experience with medically complex individuals or willingness to learn.
- Ability to exercise sound clinical judgment in routine, urgent, and emergent situations;
- Proficient computer skills, specifically with all Microsoft office applications;
- Ability to document effectively and efficiently in course of clinical work;
- Ability to navigate an electronic health record;
- Ability and desire to work as part of a clinical team while also able to be self-directed and motivated with minimal supervision;
- Ability to acquire and effectively practice new clinical skills as needed according to evolving agency needs/services;
- Demonstrate effective communication skills, organizational skills, and ability to multi-task.
Physical Demands and Working Conditions
The work will be performed in the community, client’s home, office or remotely via telehealth depending on the treatment objectives and interventions. Manual dexterity is needed to use computers. The ability to sit for extended periods of time is required. Reliable internet access, transportation, a clean driving record, and proof of auto insurance coverage is required.
