Adult Community Support Services

Adult Community Support provides a multi-faceted range of services with a goal of supporting restoration, rehabilitation and recovery for clients with severe and persistent mental illnesses such as schizophrenia, mood and depressive disorders. Our goal is to help clients attain their optimal health and live as fully as possible within their own communities. Our clinical case managers use a community-based approach, working with clients directly in their homes, workplaces and other community settings to achieve and sustain recovery, and enjoy the best possible quality of life.

As our region’s designated community mental health center, CLM provides services for the towns of Atkinson, Chester, Danville, Derry, Hampstead, Newton, Pelham, Plaistow, Salem, Sandown and Windham. Adult Community Support Services include:

Individual - Our initial goal of therapy starts with building trust so that we can communicate in a safe setting and form a healing partnership. Through this partnership, we create an environment where it is acceptable to challenge, to experience difficult emotions and to attempt new, healthier ways of living.

Group - Group therapy offers individuals a safe and comfortable place where they can work out problems and emotional issues, gain insight into their own thoughts and behavior, and offer suggestions and support to others. In addition, individuals who have a difficult time with interpersonal relationships can benefit from the social interactions that are a basic part of the group therapy experience.

Evaluation - Our team of psychiatrists and nurse practitioners are available to evaluate the need for medication or explore any medical issues that may impact mental health symptoms.

Consultation - Our medical staff works closely with our clinicians to ensure that your treatment is comprehensive and well-coordinated.

Management - Whenever possible, we will work with your primary care provider to ensure integration between your mental health and medical care.

  • Targeted Case Managers (TCM's) assess the needs of the clients in order to provide the most effective comprehensive services
  • TCM's work with the CLM team to ensure services being received at CLM are meeting the client's needs.
  • TCM's work with and coordinate often with outside entities/agencies as appropriate. The following list is only an example:

  • TCM's are an effective communication bridge between your treatment at CLM and the outside providers working with a holistic approach towards integrated care.
  • Targeted Case Management offers another layer of quality control management in an effort to assure you are receiving optimum care.
  • TCM's monitor progress with treatment.
  • TCM's assist with implementing services by providing:
  • Referrals to programs within CLM and outside-of-the-agency programs
  • Obtaining releases of information in order to communicate with other agencies/entities related to the client's goals
  • Provide information that would benefit your services at CLM
  • Targeted Case Management can be provided to clients who are covered by insurance such as Medicaid, Managed Care Companies or are willing to pay out-of-pocket.
  • Targeted Case Management is available via phone contact and/or face to face contact in order to provide flexibility and accommodate the schedule of the client.
  • Targeted Case Managers provide monitoring, referrals, linkage, and information regarding services that will benefit with your recovery.
  • Targeted Case Management is a voluntary service that is often recommended by the client's prescriber/assigned clinician.

Evidence-based Practices (EBP) are models of treatment or best practices proven effective at treating mental illness and improving clinical outcomes.

Dialectical Behavior Therapy

Dialectical Behavior Therapy (DBT) is a treatment designed specifically for individuals with self-harm behaviors such as self-cutting and/or urges to suicide. Many clients with these behaviors meet criteria for a disorder called Borderline Personality Disorder (BPD). DBT is provided through a series of group modules as well as within individual therapy sessions.

Illness Management and Recovery

CLM’s Illness Management and Recovery Program is offered in both individual and group format.Illness Management and Recovery (IMR) is an evidence-based program designed to help consumers with mental illness develop the skills and resources they need to cope effectively with symptoms and to maintain independence.

Integrated Dual Diagnosis Treatment (Mental Illness/Substance Use)

CLM runs Integrated Dual Diagnosis Treatment (IDDT) groups throughout the year for those individuals with a diagnosis of mental illness and substance abuse. The IDDT groups are group psychotherapy sessions that educate and support group participants in working towards sustained recovery from drugs and alcohol as well as successful management of mental health symptoms.

Court referred evaluations related to Substance Abuse are provided at CLM as well as DUI counseling.

Supported Employment

Supported Employment is devoted to helping consumers find and maintain meaningful jobs in the community and/or pursue education advancement for career achievement.The goal of Supported Employment is to help people with serious mental illnesses find and keep competitive jobs. SE facilitates the recovery process by supporting consumers who are interested in working or going back to school in their efforts to get on with life beyond illness people with serious mental illnesses. SE emphasizes helping people obtain competitive work in the community and providing the supports necessary to ensure success in the workplace.

Critical Time Intervention (CTI)

CTI is a free program that helps individuals getting ready to be discharged from a psychiatric inpatient setting make links in their community after they leave the hospital. Individuals are matched with a CTI Coach who will work with them to identify goals and connect with supportive people and services in their home communities. CTI Brochure

 

Functional Support Services (FSS) are interventions that may be provided to individuals meeting eligibility criteria. Functional Support Services are consumer driven goals that focus on recovery and are prescribed in their individual service plan/treatment plan. Functional Support Services must be provided in the community and not in the mental health center.

Functional Support Services include the following four services:

  • Therapeutic Behavioral Services
  • Family Support Services
  • Medication Support
  • Crisis Intervention Services

Therapeutic Behavioral Services

  • Therapeutic Behavioral Services (TBS) objectives are to develop, reinforce, problem solve, apply skills and strategies to reduce symptoms and behaviors that impede or impair the consumer's functioning and complete their individual goals.

Family Support Services

  • Family Training and Counseling (FTC) services are interventions provided to family members, care givers and significant others in the consumer's life that supports the management of the consumer's mental illness.
  • Family Supports primary objective is to increase, educate and promote the consumer's resiliency and recovery.

Medication Support

Medication Training and Support (MTS) is an intervention provided to support the consumer in maintaining their medication regimen. Medication Support educates and problem solves to develop strategies for incorporating medication into the consumer's daily routine as prescribed by the clinical/medical record to promote management of their mental illness.

Crisis Intervention Services

Crisis Intervention is delivered on an individual basis and is designed to improve acute crisis behaviors and symptoms that jeopardize the consumer's ability to remain in the community.

Assertive Community Treatment provides extensive community outreach to high risk individuals who have frequent hospitalizations, encounters with law enforcement and/or are resistant to treatment.

Supported Housing provides assistance with a goal of helping consumers live as independently as possible in the community. Support ranges from help developing appropriate independent living skills, assistance in obtaining affordable housing and ongoing support to sustain successful community integration for the program participants.

In addition, CLM continues to make significant strides through community outreach and collaboration with other community resources to address the needs of the homeless in the community. The Housing and Urban Development (HUD) funded programs offer housing assistance for eligible homeless adults, who are willing to participate in treatment and are in need of housing assistance and support.

CLM provides individual and family therapy for clients with a dual diagnosis of mental illness and a developmental disability.

CLM coordinates efforts with a client's treatment team within CLM as well as with outside providers that may be involved such as one's case manager from a regional agency, home provider, or day program staff.

The NEW You (Nutrition, Education and Wellness for a New You) program integrates mental health services and physical health education to reduce medical comorbidity and premature mortality. As a result, the NEW You program supports individuals in reducing the risk of diabetes and cardiovascular disease in addition to improving mental health symptoms, quality of life, and longevity. Participants work with a Wellness Coach on identifying personal recovery goals related to improving health and wellness and create one’s best self.

Adult Outpatient Services

CLM provides office-based treatment for adults coping with a variety of mental health issues including depression, anxiety, post-traumatic stress disorder (PTSD), grief/loss, troubled relationships, life transitions, post-partum depression and other women's issues, physical, emotional and sexual abuse, alcohol and drug-related concerns.

Our integrated approach takes into consideration the link between a client's physical health and medical conditions and how they might impact mental health treatment.

Our professionals work with the client to develop an individualized treatment plan that is solution-focused, short-term and may include one or a number of the following Adult Outpatient Services:

Family - Families function best when there are appropriate hierarchies and boundaries. When difficulties arise between family members, family therapy can be effective in re-establishing roles, boundaries and healthy communication.

Group - Group therapy offers individuals a safe and comfortable place where they can work out problems and emotional issues, gain insight into their own thoughts and behavior, and offer suggestions and support to others. In addition, individuals who have a difficult time with interpersonal relationships can benefit from the social interactions that are a basic part of the group therapy experience.

CLM provides comprehensive evaluations and treatment for substance use disorders with co-occurring mental illness. Services start with a thorough interview evaluating issues related to substance use/misuse and recommendations for treatment are developed.

Services are available to address issues related to substance use/misuse through individual or group counseling to develop coping skills and strategies and a relapse prevention plan in order to work towards and maintain sobriety.

Transcranial Magnetic Stimulation (TMS) Therapy is a breakthrough non-drug treatment for major depression that is safe and proven-effective for patients including those who have not responded to treatment with antidepressant medications.

Reiki treatments at CLM are provided by a certified Reiki master. Reiki is a gentle method of hands-on or hands-off healing that taps into the individual's energy flow. Reiki is not massage, hypnosis or a tool for diagnosing illness. Reiki is a technique that addresses both chronic and acute conditions, gently and powerfully promoting balance among all the body's systems and the regenerative processes of body and mind.

Reiki has been shown to be effective in increasing feelings of relaxation, reduction of physical and emotional pain and other symptoms of discomfort due to illness or imbalance within the body. Used by a rapidly growing number of hospitals and clinics in the United States and around the world, Reiki complements medical treatment, behavioral health services and massage therapy. Reiki is appropriate for individuals in any state of health.

Reiki appointments can be scheduled as 15 / 30 / 45 or 60 minute sessions.
